Please Attend:
CPMC Hepatitis B FREE Campaign’s SFSU Vaccination Day
San Francisco State SHS Building
October 13, 2008 10:00 AM – 4:00 PM
You must bring with you the following:
- A copy of your Confidential Communicator Message as a verification of your test results for the free vaccination
- Screening Questionnaire for Adult Immunization Form (print this form)
Please read the Hepatitis B Vaccination Information Statement prior to October 13th
You can also visit CPMC Liver Disease & Transplant Clinic located at 2340 Clay Street (3rd floor) in San Francisco every second Saturday of the month from 11:00 am - 2:00 pm or starting on April 16, 2008 each Wednesday from 4:00 pm – 6:00 pm and CPMC will provide you with hepatitis B vaccination if you can not make it on October 13th SFSU Vaccination Day.
