Required Immunizations

Prospective SFSU students should send their completed "proof of immunization form" of the REQUIRED immunizations to the Registrar's office BEFORE registration.

Failure to comply with the immunization requirements will result in a registration hold.
The Student Health Service does not generate registration holds and DOES NOT remove registration holds. For Registration Hold information please contact the Registrar's Office at 415-338-2350.

The San Francisco State University (SFSU) immunization policy requires that all students except those who were enrolled in a California public school for the seventh grade or higher on or after July 1, 1999 shall provide proof of immunization against Measles, Mumps and Rubella before being allowed to register for classes. In addition SFSU students who are 18 year old or younger will need to show proof of Hepatitis B immunization before registering for the first semester of classes.
