International Insurance Frequently Asked Questions

Does the Health Service accept payments in person for the campus insurance?

No, the Student Health Service does not accept payments for insurance. All enrollments are done through Wells Fargo Student Insurance Division. You may purchase insurance online or you can enroll over the phone by calling Wells Fargo at: (800) 853-5899 or (916) 231-3399. Both the website and phone methods accept most major credit cards. No personal checks are accepted.

 

How do I purchase the insurance for one year, online?

Go to CSU Healthlink website

  1. Click on the Enroll Today Button.
  2. Select San Francisco State University from the list and click the Submit button.
  3. Go to 09-10 San Francisco State Univ-Int’l (first choice from the options available).
  4. Click on Enroll and select the appropriate term.

 

How do I purchase insurance if I am graduating at the end of the Fall or Spring semester?

Go to CSU Healthlink website

  1. Click on the enroll today button.
  2. Select San Francisco State University from the list and click the Submit button
  3. Go to 09-10 San Francisco State Univ-Intl Midterm Grad (second choice from the options available)
  4. Click on Enroll and select the appropriate term

When will my hold be removed once I have purchased insurance online?

Always print out your receipt when you purchase the campus insurance through Wells Fargo. Your online purchase will automatically generate an email to the Registrar’s office telling them to remove your hold. It may take up to several days for the Registrar to clear your hold.

 

I have my own insurance, is that okay?

If you chose NOT to purchase the campus insurance you must have your insurance company fill out a Health Insurance Affidavit on their letterhead (forms are available online in Microsoft Word and PDF format). Please fax the forms to (415-338-2278) or send it as an attachment in an email to the Student Health Services (email address: shsinsur@sfsu.edu) and allow 10 business days for the Affidavit to be reviewed by Student Health Services.

 

If I purchase the campus insurance how much is it?

Refer to the web site for cost of the annual and the semester plans. (The insurance company is Blue Cross)

 

While purchasing insurance online, it asks me for a local address and phone. If I don’t have an address or phone in the USA, what do I put in the required fields?

You can use the SFSU campus address: 1600 Holloway Avenue, San Francisco, CA 94132 until you get a local address. Unless you have a cell phone that will work in the USA you can simply use 123-456-7890 (no one will have that number). When you get a local address and phone, please call Wells Fargo customer service (800-853-5899) to update your information or you can log into your account on the CSU healthlink website to update your address; just use the email and password that was created during your enrollment process to log in.

 

What if I don’t have my insurance card yet, but I need to see a doctor?

You can print out a temporary ID card after completing your insurance purchase (on-line) to use until you receive your insurance card. Cards are usually mailed out within three weeks after you have purchased insurance, for new enrollees only. Students who re-new their policies will not get a new card.

 

Will Blue Cross pay me back if I have to pay with my own money for medication or visits?

If the visit or medication was done at the Student Health Services, you will have to fill out a claim form and send it in to Blue Cross along with official receipts to get your money back. You can log on your SHS account and print an itemized statement of your charges to mail with the Blue Cross claim form (PDF). Mail your form and receipts to:

If the visit was not from the Student Health Services, usually most health care providers will bill Blue Cross for you.
