How to Document Your International Health Insurance Coverage

Option 1 – CSU Sponsored Insurance

If you have purchased the CSU-sponsored policy, Wells Fargo Student Insurance Division will automatically notify the SFSU Registrar’s Office to release your insurance hold with each purchase. Online purchases will generate a notice immediately. During peak or busiest periods, it may take the Registrar up to one working week to release your registration hold.

 

Option 2 – Other Health Insurance

For any other insurance, provide proof of your coverage to Student Health Services for approval.

“Proof” entails:

  • Completion of the Health Insurance Affidavit (F-1 visa or J-1 visa)
  • Affidavit must be completed on Insurance / Employer company letterhead. By signing the affidavit, the insurance company or employer, through its representative, assumes all responsibility for appraising compliance. SFSU may confirm the company data provided. Submission of false documents could result in the student’s expulsion and possible deportation.

If you are unable to obtain an Affidavit from your insurance company or employer, we will have a professional broker review your policy. This process could take up to three weeks. (see Lead Time Required to Clear Hold)

Please be sure to provide an email address or continental USA phone or fax number. Your coverage must include health and emergency transportation insurance (medical evacuation and repatriation). If you have a US health policy, it will not include medical evacuation and repatriation. SFSU assumes no responsibility for any medical, evacuation or repatriation expenses incurred whether you have insurance or not.

 

Note: Duration of required coverage may differ for some individuals especially visiting international scholars
