No Show Fee
Students, who need to cancel their appointment can do so by e-mailing OR calling (415) 338-1251 the SHS cancellation line in advance of their scheduled appointment.
Due to high patient demand and the limited availability of appointments, we have instituted a $20 no-show fee. You must give advance notice to cancel and appointment or you will be charged a $20 no-show fee (email cnclappt@sfsu.edu, call 415-338-1251). The prepaid fees for the following services will be forfeited in lieu of the no-show fee:
Podiatry Clinic $40
Remember all “no shows” and late cancellations will be billed $20.00.
Petitions for a "No Show / Late Cancellation Fee Waiver” will only be approved for significant bona-fide reasons.
Petitions for a "No Show / Late Cancellation Fee Waiver” will only be considered if received within one month of your missed appointment.

On the Subject Line please enter - Appt Cancellation
In the body please enter - your First Name, Last Name, Student ID number, and the date and time of the appointment you wish to cancel.
Telephone Cancellation Instructions
Telephone 415-338-1251 Press #1 on the keypad.
Please leave a message stating your First Name, Last Name, Student ID number, and the date and time of the appointment you wish to cancel. Your message will be date and time stamped.
Petition Process
If you have a significant reason(s) for having missed your appointment, please send the completed "No Show / Late Cancellation Fee Waiver Form” (Word format) to the SHS via:- Your SFSU email address to shssa@sfsu.edu.
On the Subject Line please enter - No Show Fee Petition
OR
send it through the US mail to the SHS address listed below.
Please be advised that it may take 5 working days after receipt to process your petition. The reply will be forwarded to your SFSU e-mail address.
