Medical Records Request
You may mail or fax your request to:

To request medical records, please send via mail, fax or bring it to the SHS. All requests must be signed.
Your request must contain the following:
- Full name
- Your SFSU University ID number
- Birthdate
- Years you attended SFSU
- Complete name, address, zip code of the doctor or clinic from which you are requesting your medical records be sent to or received from.
- Be specific in what information you would like sent to or from the SHS
- You may fax your request to (415) 338-2278
- Process takes approximately 7 - 20 days business days.
- If your release form is missing information, this will delay the process
Record Release Forms are available at the Student Health Services during operational hours or can be mailed.
If you have general questions, please call (415) 338-7333. If you wish to leave a message for the Custodian of Records, please call (415) 338-1981. There may be a fee for the service, please speak directly to the Custodian of Records.
For copies of your Immunization Records, if you received your immunizations at the Student Health Services within ten years of your last visit, we will provided verification.
