By signing this form, I grant the San Francisco State University (SFSU) College of Science & Engineering (COSE) permission to check SFSU’s records to verify these facts.
Criteria:
1. Students must have a declared major in
the College of Science & Engineering.
2. Students must be enrolled at least half-time
in Spring of 2003.
3. Students must have a minimum, cumulative
GPA of 2.0 (undergrads) or 3.0 (grads).
4. Students must complete a minimum, cumulative
of 100 hours of CS/CSL during the time they have been enrolled at SFSU.
5. There shall be no limitation to the qualifying
service in term of activity or intent. However, activities more closely
related to the areas of science and engineering will be given greater consideration.
"Community Service"
Definitions:
. Community Service
(CS) is defined by any unpaid volunteer service which contributes to the
well being of the community in which the effort is directed. Most
typically, community service shall be defined as an effort motivated primarily
by selfless intent and a desire to give to the community.
. Community Service
Learning (CSL) is the integration of community service with academic learning
so that each is enhanced by other. (These are now more than 120 courses
at SFSU that offer the opportunity to participate in CSL.)
Application Procedure:
1. A completed Community Service Scholarship
application (this page).
2. Proof of completion of the 100 CSL experience
hours.
3. A short essay of 400-600 words detailing
your CS/CSL experience.
4. A recommendation from one supervisor of
the CS/CSL experience (either written or via e-mail to science@sfsu.edu).
5. An unofficial current copy of your SFSU
academic transcript.
Evaluation Weights: Essay (50%), Recommendation (40%), Academic
Record (10%)

an essay, one recommendation, proof of CSL experience, and an unofficial
