By signing this form, I grant the San Francisco State University (SFSU) College of Science & Engineering (COSE) permission to check SFSU’s records to verify these facts.

Criteria:
1.  Student must be enrolled in one of the Masters Degree Programs in the SFSU College of Science & Engineering.
2.  Student must have completed a minimum of 12 graduate units at SFSU.
3.  Student must have a strong record of academic achievement.
4.  Student must demonstrate a financial need.

Requirements:
1.  A completed COSE Advisory Board Scholarship application (this page).
2.  A short essay of 400-600 words covering the following topics: a) How you will use your degree to better the community upon graduation, b) Reasons for applying for this scholarship, and c) Your financial need.
3.  E-mail recommendation from TWO SFSU professors in your field of study to science@sfsu.edu
4.  A current unofficial copy of your SFSU academic transcript.                   

Evaluation of applicants will be based on the essay/paragraph, faculty recommendation, and academic record.

Application Deadline:
Bring the completed packet (an application, an essay, two recommendations, and an unofficial transcript) to the COSE Office of Alumni Relations & Student Projects, Science 248, or mail to: San Francisco State University, College of Science & Engineering, 1600 Holloway Avenue, San Francisco, CA 94132-4163, Attn: Lannie Nguyen-Tang, Scholarship Committee.

APPLICATIONS MUST BE RECEIVED
BEFORE 12 PM (NOON) ON MONDAY, OCTOBER 18, 2004. 
POSTMARKS WILL NOT BE ACCEPTED.
