State Vehicle Use Policy

All authorized state drivers must be an SFSU employee or an SFSU appointed volunteer. Students or employees of the SFSU Auxiliary are not allowed to drive on state business. These requirements apply to individuals driving in a state-owned, state rented, or privately owned vehicles while on State business.

  1. A valid California drivers license
  2. A current Defensive Drivers Training certificate
  3. A good driving record  (Employee Pull Notice program)
  4. A completed STD 261 Authorization to Use Privately Owned Vehicles on State Business form if using a privately owned vehicle for state business.  This form must be submitted to the person authorized to approve travel expense and is renewed annually.

Designated vehicle drivers must obtain University approval and meet all the requirements of the University's Driver Safety Program (DSP) before driving for official State business. The Office of Environmental Health and Occupational Safety (EHOS) is delegated the responsibility of authorizing official University drivers. Contact EHOS for further information at (415) 338-1449.

State employees may also reference the CSU's State Vehicle Use Policy manual called the CSU: Use of University and Private Vehicles Policy Guidelines located on the CSU system wide website.

Class Field Trips

Everyone going on a class field trip is required to be pre-approved by the instructor.

 

COSE Vehicle Policy

The College of Science and Engineering has a small fleet of vans available for field trips and work-related business. All drivers must meet requirements outlined in the State Vehicle Use Policy above and includes the following additional requirements:

 

Reserving a Vehicle

Contact Miriam Knof in the Geosciences Office (x8-2993) to reserve a vehicle once you have department Chair apprpoval and meet all the requirements. Biology faculty and staff should contact Michael Fong in the Biology Stockroom (x8-7678) for assistance in obtaining Biology Dept. Chair approval.

Reporting Vehicle Accidents

Determine whether immediate medical attention is needed for anyone involved in the accident. Obtain necessary medical assistance.

  1. Use the STD 269 (Accident Information Card) in the glove compartment of the State vehicle and share your vehicle and drivers license information with the other driver. Do not admit any fault or make promises.
  2. If any police authority or California Highway Patrol (CHP) is present, obtain a police report number if one is completed.
  3. Report any accident to the Office of Risk and Insurance Management (ORIM) that involves any State-owned vehicle or any vehicle used for State business within 48 hours. Complete and submit the form STD 270 (Vehicle Accident Report) to your Supervisor and the campus Office of Risk Management. If a police report was completed, provide the report number with the STD 270. Your supervisor must complete and submit form STD 274 (State Driver Accident Review).
  4. Immediately report any bodily injury or significant property damage to the State Office of Risk Management and Insurance (ORIM) by telephone at (916) 376-5302 or by FAX (916) 376-5277. Weekend voicemail messages may be left at their toll free phone number (800) 900-3634. All calls will be returned next business day.

Please note that an accident does not require reporting to the State Office of Risk Management and Insurance (ORIM) when damage is less than $500 and no other party is involved.
