1. Tell the Doctor or Healthcare provider you are a SFSU employee.

2. Tell the Doctor or Healthcare provider you were injured and the nature of your injury / illness.

3. Tell your supervisor about your injury / illness immediately.

4. Get a Workers' Compensation Claim form (DWC Form 1) from either:

• Your supervisor
• Safety and Risk Management (ADM 252)
• Student Health Center (SHC)
