Authorization to Use Privately Owned Vehicles on State Business form STD 261

The STD 261 Authorization to Use Privately Owned Vehicles certification form verifies the employee's private vehicle has minimum amounts of liability insurance coverage, is in safe mechanical condition, and is adequate for work use in accordance with the State Administrative Manual (SAM) section 0753. This form advises the State employee that adequate vehicle insurance coverage must be provided by the employee's personal insurance and that personal vehicle insurance is primary coverage in case of vehicle accident.

The supervisor authorized to approve travel shall retain the STD 261 completed authorization form in the employee's personnel file. The STD 261 authorization form is valid for a period not to exceed one year and may be initialed and dated annually by the authorized employee driver and employee's supervisor to certify adequate personal auto insurance is current and that the private vehicle is safe for State business use. Accident Information Card form STD 269

The STD 269 Accident Information card is used to record all pertinent information before leaving the scene of the when when the driver of a University or State-owned vehicle is involved in an accident . Blank STD 269 cards should be readily available in the glove compartment of each State (rental, owned, leased, or private) vehicle. When another vehicle is involved in an accident with a State vehicle, the appropriate portion of the STD form 269 is completed, detached, and given to the driver of the other vehicle. The State Office of Risk and Insurance Management (ORIM) is the University's third-party auto insurance provider.

Vehicle Accident Report form STD 270

The STD 270 Vehicle Accident Report form is used to report all State vehicle accidents within 48 hours to the State of California Office of Risk and Insurance Management (ORIM). If the accident results in bodily injury or significant property damage, the accident must be first reported by telephone and/or fax copy of the STD 270 to the ORIM fax phone number at (916) 322-6006.  The ORIM website allows individuals to download a copy of instructions to complete the STD 270 Vehicle Accident Report.

The STD 270 Vehicle Accident Report must be accompanied by the STD 274 State Driver Accident Review form completed by the employee driver's supervisor be submitted within 48 hours to the State of California Office of Risk and Insurance Management (ORIM).

A copy of the STD 270 and STD 274 should also be forwarded to the campus Office of Risk Management office located in the Corp Yard, Suite 201 or via fax (415) 338-2498.

State Driver Accident Review form STD 274

The STD 274 State Driver Accident Review form must be completed by the supervisor of the employee driver involved in an accident and must accompany the STD 270 Vehicle Accident Report completed by the employee driver within 48 hours to the State of California Office of Risk and Insurance Management (ORIM). The STD 274 State Driver Accident Review form is a record or the employee supervisor's prompt and through investigation of the State vehicle accident. The supervisor must initiate any verbal or written corrective action with the employee driver, record the action on the STD 274, and forward copies of the completed STD 274 form along with the STD 270 to the campus administration (the SFSU Office of Safety and Risk Management). The purpose of the STD 274 State Driver Accident Review form is to determine whether the accident was avoidable and to aid in the prevention of future State vehicle accidents.

A copy of the STD 270 and STD 274 should also be forwarded to the campus Office of Risk Management office located in the Corp Yard, Suite 201 or via fax (415) 338-2498.

Enterprise Rent-A-Car FAX Authorization Forms

State contracted auto liability coverage applies only when the employee uses the uses the American Express (State) Government Card card for rental car payment. The exception to this payment rule is a direct billing arrangement authorized by the Enterprise Rent-A-Car vendor on campus (Local FAX Authorization, Traveling FAX Authorization). Cash, personal checks and personal credit cards are prohibited forms of payment for State contracted commercial car rentals.
