Password Change Procedure

The president of the student organization,with an SF State student I.D., is responsible for completing and submitting the form. This form will be processed within 10 business days. An incomplete form will delay the process.
 
  1. Go to Password Change Form.
  2. Input information and print form.
  3. Turn in your completed form to OSPLD with student identification.
