New, continuing and re-activating student organizations must register annually with OSPLD. The registration forms for 2008-2009 academic year are available at the links below; the forms may be opened by using Adobe Acrobat Reader. Please note that there are two separte forms, one for general student organizations and one for sororities/fraternities. The deadline for registration is September 30th. Organizations that do not register by this date will be considered inactive and unregistered.

Print the form after completing all required fields. Signatures from President, Financial Officer and other members/officers are required. Once the form is complete with all signatures, return the completed form to the OSPLD office
