What Forms Do I Need?
Form 4868 is for obtaining an extension for filing your Federal income tax forms. Form FTB 3519 is for obtaining an extension for filing your California State income tax forms. Both forms may be downloaded from:http://www.irs.ustreas.gov/pub/irs-pdf/f4868.pdf

Who Should Use These Forms?
If you will not be able to file your tax returns by the April 18, 2011 deadline (either because you weren't able to complete all the paperwork OR because you are lacking important required forms, such as form W-2 or 1042-S) then you must file form 4868, and, if necessary, FTB 3519 on or before April 18.Forms 4868 and FTB 3519 provide an automatic six month extension. However, form 4868 and FTB 3519 do not allow for an extension of taxes owed. If you owe tax to the U.S. government or the State California, you must enclose payment (or an estimated payment) with the appropriate form.
Filing Instructions:
For Form 4868, Complete all items in Part I. In Part II, line 4, enter the amount from form 1040NR EZ line 17 or form 1040NR line 59. In Part II, line 5, enter the amount from form 1040NR EZ line 21 or form 1040NR line 68 (excluding line 63). In Part II, line 6, subtract line 5 from line 4. If line 5 is more than line 4, enter "0". Line 7 should indicate the amount of tax payment you are enclosing with this form (if any).Make a copy of each form BEFORE you mail it in, and send form 4868 to:
Internal Revenue Service Center P.O. Box 660575 Dallas, TX 75266-0575 if you are enclosing a payment for taxes due OR to Internal Revenue Service Center, Austin, TX 73301-0215 if you are not enclosing a payment.
Mail form FTB 3519 to: Francisce Tax Board, P.O. Box 942867, Sacramento, CA 94267-0051.
Do not attempt to file the 4868 or FTB 3519 electronically... file it as a paper form.
