MUSIC SCHOLARSHIP APPLICATION
(for all Music Department Scholarships)

Download or request a scholarship application (see below).

Although live auditions are strongly encouraged, a scholarship application may be sent with a recording of your performance. Applications with tape must arrive at SFSU no later than the date of scholarship auditions.

Scholarship applications must be filled out, signed, and returned to the Music Department with a copy of your transcripts (unofficial is acceptable). Note that these are in addition to the transcripts required by the Office of Admissions.

Download either the PDF version or the Microsoft Word version of the application. The documents can either be filled out on your computer and then printed and signed, or printed first and then filled in by hand. (Please type or print clearly). A scholarship audition can only be scheduled by telephone; call 415-338-1431.
