Risk Management

The Community Service Learning Program has adopted the CSU’s guidelines for managing risk in Community Engagement and the following forms for Faculty, Students and Community Partners are to be used with the process of student placement. Please note that some of these forms are required.

Faculty Forms

The following are forms for faculty interested in developing community engaged opportunities or are teaching Community Service Learning courses. All documents are in PDF format.

 

  • SF State CSL Project Planning Checklist-Required if Faculty is doing their own agency site visit. Faculty who conduct their own agency site visits are required to use this form designed especially for them. It is course specific and will help faculty review requirements for both the learning site and for students. Reviewing how the site envisions student learning can prevent future issues; faculty will also have a better understanding of students risks, logistics, and supervision.

 

Community Engagement Course Forms (Faculty)

 

  • Community Engagement Learning Plan (Required) This is the official document, which describes the students' specific responsibilities at their service learning site. The student, course instructor, and site supervisor must sign this document.

 

  • Student Sign-in Sheet This form is provided to assist students in tracking the activities and number of hours they served at their service learning site. However, students should track their hours in SF State’s ULink49 web-based database. Students should submit their hours on ULink49, make sure that their site supervisor approves the hours, and they should notify their service learning professor that hours have been verified and approved by the site supervisor. Once completed and approved by their site supervisor, it provides verification of their service to the organization as well as a history of their contributions to the site.

 

  • Student Performance Evaluation This form is used at the discretion of SF Sate course instuctor. Site supervisor will complete this assessment of student's performance.

This video uses images, interviews, and text to provide information on the City's Diverse neighborhoods and cultures, background on how to work with communities respectfully while doing community engaged work, and provides clarity about student's rights/responsibilities, as well as faculty expectations around civic engagement and leadership development.

Student Forms

The following are resources for students taking a CSL course at SF State.  All documents are in PDF format.

 

  • Community Engagement Learning Plan (Required) This is the official document, which describes your specific responsibilities at the service learning site. The course instructor, site supervisor, and you must sign this document.

 

  • Student Sign-in Sheet This form is provided to assist you in tracking the activities and number of hours you served at your service learning site. You should submit your hours on ULink49, make sure that your site supervisor approves the hours, and you should notify your service learning professor that hours have been verified and approved by the site supervisor. This web-based database (ULink49) provides verification of your service to the organization as well as a history of your contributions to the site.

 

  • Student Performance Evaluation This form is used at the discretion of SF State course instructor. Check with your instructor to see if they require this form and the due date. Your site supervisor will complete this assessment of your performance. Your service learning course professor will consider the remarks of this evaluation in determining your final grade in your service learning course. You are responsible for having your site supervisor complete this form after verifying your Activity and completed CSL Hours.

 

  • Protocol for Termination of Students This protocol can be used by the community site to terminate your "volunteer" work with their agency.

All SF State Students participating in community engaged work are required to watch this 13 minute video and download the Certificate of Completion-Video form acknowledging that they have done so.  This form needs to be returned to the course instructor.
