Introduction
- The Injury and Illness Prevention Program (IIPP) is required by California Senate Bill 198 and implemented under the California Code of Regulations, Title 8, Chapter 4, Section 3203. IIPP serves as the umbrella program for all health and safety programs.
- The President of the University has ultimate responsibility for the maintenance of a safe and healthful workplace.
- Deans, Directors, Department Heads, Department Chairpersons, Principal Investigators and supervisors are accountable for establishing and maintaining programs to ensure IIPP compliance.
- All employees are responsible for knowing the applicable safety regulations governing the activities they carry out and are accountable for complying with them.
Authority and Responsibility
- The Department Head or Chairperson has the responsibility and authority for implementation. They may elect to delegate the authority for implementation for the Program to someone in their Department but they may not delegate their responsibility for its implementation (Appendix A must be completed and forwarded to EHOS).
- Department Safety Coordinators serve as liaisons between EHOS and campus departments.
Compliance with Safe Work Practices
- The Code of Safe Practices, developed by management or supervisors, is the instrument employed to instruct and define for employees in a specific functional environment, what protective measures must be taken to avoid most workplace injuries (see Appendix B).
- Employee Training Checklists for General Safety (Form IIPP-1) and for Shop/Laboratory Safety (Form IIPP-2) can be used to track employee training.

Procedures For Investigating Injuries and Illnesses
- Follow procedures outlined in the “Notice To Employees…When a job injury occurs”, Appendix J.
- Be familiar with Worker's Compensation Forms- DWC Form 1 and Form 5020 (see Appendix G, H).
Procedures for Correcting Unsafe or Unhealthy Conditions
- Periodic inspections (EHOS annual and Department Semi-annual audits) provide a method of identifying potential hazards.
- Hazards should be reported immediately to supervisors and/or Safety Coordinators.
- Hazard Reporting Form (IIPP-7) may be used to report hazards.
- If corrective efforts are unsuccessful or beyond the capability of the college or department or beyond the capability of campus resources, the supervisor and/or the Safety Coordinator will report the problem to EHOS.
Safety and Health Training
- Training must be documented.
- Employee Training forms for General Safety (Form IIPP-1) and Shop/Laboratory Safety (Form IIPP-2) or any other similar forms can be used to document training.
- Available resources include the EHOS Video Library, Chancellor's Office Video Library and EHOS Provided Training Sessions.
Record Keeping and Documentation
- The following forms, or their equivalent, may be used for documentation:
- Record Retention
- Training, Inspections-one year
- Environmental monitoring records, employee exposure records and other medical records-duration of employment plus 30 years
