Online Clearance Request


Use this form to initiate the clearance procedure. The Human Resources Clearance Specialist will begin the clearance process once notification is made. It is advised that you turn in all library books, Audio/Visual Equipment, travel advances and pay cashiers for any lost keys, property, etc. (Please retain any receipts as proof).

Please bring all SFSU issued keys, ID’s and parking permits to your appointment. If you do not have any of these items to turn in, an in-person appointment with the Clearance Specialist is not necessary.

Otherwise, appointments are made on a first come, first serve basis. If your requested appointment is not available, the Clearance Specialist will contact you to reschedule. All appointments will be confirmed via E-mail or by phone. Please call *54359 for any further questions. We are located in the New Administration Building, Room 252.
