Leave of Absence Request (Educational or Health)
The application for Leave of Absence Request (Educational or Health) must be signed by your advisor and department chair and submitted to the GradStop Counter before the end of the semester prior to your absence. The petition must demonstrate that you will be doing something significantly relevant to your graduate program.
