Application For Graduate Degree

This form is available and may be submitted only by students who have an approved GAP on file by the appropriate deadline. This form is filled out online and then MUST BE PRINTED. The form must be signed by you and by your advisor or department chair before being submitted with the $40 application fee. READ ALL INSTRUCTIONS CAREFULLY. Please pay by cash or check at the Cashier's Office prior to submitting your form to the Division of Graduate Studies. For students who mail in their signed and approved forms, please attach a check or money order made out to SFSU and send with the application. Please call the Graduate Hotline (415) 338-2234 if you need assistance. Summer graduates who wish to attend Commencement in May, can do so by selecting the “Spring/Summer” term.

Request For Reissue Of Diploma Form

Use this form if you are requesting: (1) a second diploma (reissue), (2) a name change on your diploma, or (3) a retroactive degree. Submit this form to the Division of Graduate Studies, along with a check made payable to SFSU in the amount identified on the form.

Certificate Approved Program

To apply for award of certificate, please submit the form to your Certificate Advisor and Program/Department Chair for approval. You will need to pay a $7.00 processing fee at the Cashier's Office before submitting the form and your unofficial transcripts to the GradStop Counter (Administration Building 250).
