Forms for Continuing Students Special Instructions

Academic Standing Petition (Probation / Disqualification)

For students on Academic probation, petition must be filled out online and printed for signature. Please submit this form to your department and college offices for approval before submitting the form to the Registrar to clear your Registration Hold. * Please include an unoffical copy of your transcript with your form.

Application For Graduate Degree

Apply Online

This form is available and may be submitted only by students who have an approved GAP on file by the appropriate deadline. This form is filled out online and then MUST BE PRINTED. The form must be signed by you and by your advisor or department chair before being submitted with the $40 application fee. READ ALL INSTRUCTIONS CAREFULLY. Please pay by cash or check at the Cashier's Office prior to submitting your form to the Division of Graduate Studies. For students who mail in their signed and approved forms, please attach a check or money order made out to SFSU and send with the application. Please call the Graduate Hotline (415) 338-2234 if you need assistance. Summer graduates who wish to attend Commencement in May, can do so by selecting the “Spring/Summer” term.

Certificate Approved Program

To apply for award of certificate, please submit the form to your Certificate Advisor and Program/Department Chair for approval. You will need to pay a $7.00 processing fee at the Cashier's Office before submitting the form and your unofficial transcripts to the GradStop Counter.

Graduate Approved Program (GAP)

This form must be filled out and printed for signature. No hand-written forms will be accepted. Please submit this form to your department by the appropriate deadline for program/department approval. A current unofficial transcript must be attached to the form.
*Must be filed at least one semester before the semester of graduation. Please include an unofficial copy of your transcripts with your form. " Check deadline published in the university calendar.

GAP Substitution or Committee Revision

To make any changes to your Graduate Approved Program (GAP) or Culminating Experience committee, please fill out and print this form for signature. Submit the form to the GradStop Counter when all appropriate signatures have been obtained. A current unofficial transcript must be attached to the form.

Petition for Extension of the Seven Year Limit

The petition for Extension of the 7 Year Limit requires supporting documentation. It's recommended that you download the petition form and request an appointment with your graduate advisor or department chair to assist you. Incomplete petitions will be returned to the student until all supporting documentation can be provided. This petition is a one-time only extension if granted. No further extensions will be given. Please allow 4-6 weeks for processing.

Planned Educational Leave Form

The application for Planned Educational Leave must be signed by your advisor and department chair and submitted to the GradStop Counter before the end of the semester prior to your absence. The petition must demonstrate that you will be doing something significantly relevant to your graduate program. Leaves are NOT granted for health or other personal reasons.

This form is issued by the academic department only. The Division of Graduate Studies does not give this form out to students. Please contact your graduate advisor or program/department office for assistance.

Request for Change of Graduate Program

Please attach an unofficial copy of your transcripts to this form before submitting to the Academic Department for appropriate signature. The Academic Department will fill out the remaining portion of this form and return it to the Division of Graduate Studies for processing.
*Students who have not completed at least one semester of graduate studies at SFSU and want to submit a Request for Change of Graduate Program/Major can do so after the fifth week of instruction.

Request to Exceed Regular Academic Unit Load
