ATC Substitution

 

Please provide all requested information for changes to no more than 3 courses listed on your original ATC form. If you need to remove or add more than 3 courses, please fill out a new ATC form instead.

 

Type and print out the form and obtain the required signatures. Handwritten forms will not be accepted. Attach a current unofficial transcript and submit to the GradStop, ADM 250.
