On completion of the internship, students must submit a comprehensive report to their department. An oral defense is required. You are required to submit a Report of Completion to the Division of Graduate Studies.

This form must be completed online and printed for signatures. No handwritten proposals will be accepted. Please be sure to include the department, name, and rank of your faculty member (professor, associate professor, assistant professor or lecturer). If you do not know the rank, ask your department office staff or the instructor.

Complete the “Description” section carefully so that your intentions are clear, especially if you plan to use human or animal subjects (see information on Human or Animal Subjects). Do not attach a copy of your research or project proposal that you may have completed for a research class. The proposal you write for a research class may be different from a human or animal subjects protocol that is required if you plan to use human or animal subjects (Some instructors have students prepare proposal in protocol format).

Please note: You must have an approved Graduate Approved Program (GAP) and an approved Proposal for Culminating Experience on file with the Division of Graduate Studies in order to register for your Culminating Experience course.
