New Hires and Continuing Students
Work-study Clearance Form (example)
Prior to the first day of employment, work-study students must submit a Work-Study Clearance Form to the employer. Clearance Forms are generated by the Office of Student Financial Aid and are mailed to the student shortly after the student registers for classes through Touch Tone in the Fall and throughout the academic year. Students who do not receive a Clearance Form in the mail can go to the Student Service Center to resolve issues that prevented one to be mailed. To obtain a Clearance Form, the student must be enrolled in full-time units (12 undergraduate, 8 graduate) and must have completed the Federal I-9 process. Clearance Forms must be completed by the student and the employer and the original returned to the Office of Student Financial Aid immediately to avoid a hold being placed on the student's paycheck. The student must obtain a Clearance Form for each employer.
I-9 Federal Employment Eligibility Verification Form (example)
This process requires the student to present various original documents for verification of the student's eligibility to work in the United States based on the Immigration Reform Control Act of November 6, 1989. Students are informed of the appropiate documents when they receive their Clearance Form.
SPAR (Student Payroll Action Request Form) (example)
Departments should have these forms available for the student to complete. These forms are available from Corporate Express at 1-800-760-3280 - Form SFO911. The Office of Student Financial Aid does not provide SPAR forms to students or employers. Completed forms are submitted to the Payroll Office in order to enter the student on the State Payroll System.
