Below is the list of forms that you can dowload or print and fill out to clear your hold or to change your financial aid information to our office. Each form below includes a description of its purpose. Read the instructions on the form carefully and submit with documentations to the Financial Aid Office.

Description & Purpose |
| Identity Verifications Form | Only use this form if you have been directed to do so by the SF State Financial Aid Office. Attach appropriate documentation and submit in person or by mail to our office. |
| Satisfactory Academic Progress Appeal | Only use this form if the Financial Aid Office has notified you that you are out of compliance with the Satisfactory Academic Progress guidelines. If you aren't sure if this applies to you, contact the Financial Aid Office. |
| Selective Service Compliance | Go to the Selective Service web site to obtain verification of your registration, register if appropriate, or obtain written advisory opinion from the Selective Service's Office of General Counsel. |
| Confirm Citizen Status Form (attach appropriate documentation required) | This form is used when proof of U.S. citizenship is being required. Only use this form if you have been directed to do so by the SF State Financial Aid Office. Additional documentation is required. You can mail or bring the forms in person; fax is not acceptable. |

You must discuss your case with a Financial Aid Counselor before you may complete these forms. Read the instructions carefully, and provide the Counselor's name with whom you discussed your situation. Incomplete forms will not be processed |
