Apply Online Complete the SFSU application at CSU Mentor-- www.csumentor.edu Apply online for EOP by filling out the Applicant Information Form located on the CSU Mentor website. Have two different teachers/instructors, counselors, or community program advisors fill out the Recommendation Form. Apply by Mail Applications can also be downloaded and printed from the link below. Remember to submit TWO Recommendation Forms. Mail completed Applicant Information and Recommendation Forms to: San Francisco State University
