    UNDERGRADUATE ADMISSIONS ASSISTANCE
    FOR STUDENTS WITH DISABILITIES
    Fall 2008 and Spring 2009

    STUDENTS WITH DISABILITIES NEED TO APPLY FOR ADMISSION TO THE UNIVERSITY USING THE STANDARD ADMISSIONS PROCESS.

    The admissions requirements for freshmen and transfer students can be found online at: http://www.sfsu.edu/prospect/. If your disability has prevented you from meeting any requirements and you believe you are an “otherwise qualified applicant,” you may be eligible to request alternative admissions assistance from the Disability Programs and Resource Center (DPRC). On a case-by-case basis , the DPRC examines an applicant’s documentation and academic record to determine whether or not to support a recommendation for alternative admissions. IMPORTANT NOTE: Not all deficiencies can be supported.

    STEPS TO APPLY FOR ALTERNATIVE ADMISSIONS ASSISTANCE FROM THE DPRC

    1. Apply to the University online at http://www.sfsu.edu/prospect/.

      If you need assistance with the application process, please call us at 415/338-2472 V/TTY to set up an Application Assistance Appointment. You must bring any unofficial transcript(s) to that appointment for us to be able to assist you with the CSU Mentor on-line application.

      All undergraduate admissions applications must be submitted by:
      Spring 2009 – Admission applications will be accepted from August 1-31, 2008

    2. Submit/request to send official transcript(s) to the Admissions Office immediately after you apply for admission. You must follow up and verify that your official transcript(s) were received by SFSU. Please check your online application status weekly to verify that all paperwork requested by SF State's Admissions department was received: https://www.sfsu.edu/online/appstat.htm.
    3. IMPORTANT NOTE: We cannot offer Alternative Admissions Assistance to prospective students who DO NOT complete a University Admissions application and submit all official transcripts to the Admissions office prior to the DPRC deadline. Do NOT follow Admission’s Department transcript deadline.

    4. Submit the following information below to the DPRC:
      a. DPRC registration form:
      b. Full documentation of your disability per the DPRC guidelines (contact the DPRC at 415/338-2472 v/tty for registration form and disability guidelines)
      c. Unofficial transcript(s) from previous school(s)* (official transcript(s) submitted to the Admissions Office are NOT available to the DPRC)
      d. Letter of support from a counselor or disability specialist following the guidelines stated below.

    All DPRC documentation in number 3 above, must be received at the DPRC by the following dates for paperwork to be processed:

    Spring 2009 applicants – deadline is Wednesday, October 1, 2008


    We cannot process your Alternative Admissions Assistance request for prospective students who have NOT completed an Admissions application and submitted all official transcripts to the Admissions office.

    *If you are an upper division transfer student who cannot meet the University’s quantitative reasoning requirement, you must provide a transcript showing that you attempted appropriate math classes at least twice in the proper sequence (e.g. pre-algebra, beginning algebra, etc.) and that you used disability accommodations on at least one attempt. Exceptions to these criteria may be considered on a case-by-case basis.

    Documentation will not be processed until all of the above steps have been completed. Once we have all of the above information, have received confirmation from Admissions of any deficiencies and if deemed necessary interviewed you by phone or in person, we will notify you of our decision to support/not support your admission by exception.

     

     


    Guidelines for Counselor/Disability Specialist
    Letter of Support For
    Alternative Admissions Assistance
    At San Francisco State University

    An admissions support letter from a counselor or disability specialist with whom the student has recently worked is an important part of the process of evaluating a request for alternative admissions assistance.

    The support letter must contain the following information:

    1. A detailed description of the specific way in which the student’s disability prevented him/her from completing the requirement; general statements are not sufficient.

    2. Detailed information about the attempts the student has made to meet the requirement, including accommodations and services used. If accommodations and services were not used, please explain.

    3. An explanation of how the student can be successful in a degree program at SFSU even though he/she has not completed the requirement. Information about other areas of academic success and discussion of how the admissions deficiency will or will not impact the student in his/her major are helpful here. (Please note that the University is not required to alter academic requirements that are essential to the major being pursued.)

    Completion of your application for alternative admissions assistance does not automatically result in a DPRC recommendation for admissions. Decisions to recommend admission are made on a case-by-case basis.

    All the above DPRC documentation must be submitted to the Disability Programs & Resource Center (1600 Holloway Ave., San Francisco, CA 94132-4046) by Wednesday, October 1, 2008 for the Spring 2009 semester.
