San Francisco State University's Robert Westwood Scholarship

Application for Fall 2012

The intent of this scholarship is to assist SF State students who are living with HIV and have made contributions or plan to use their studies contribute to communities affected by HIV. Contributions can be in any field, including: the arts, health sciences, social services, etc. One $1,000 scholarship will be awarded at the beginning of the Fall 2012 semester. The recipient must be enrolled in Fall 2012 to receive payment.

To apply, your packet must include the following:

  1. A copy of your most recent SF State academic transcript (unofficial). You must be enrolled at least half time (6 units minimum) both smesters with an SF State GPA of at least 3.0.
  2. Verification from your physician that you have HIV (a note on official letterhead or from SF State Disability Programs & Resource Center is sufficient).
  3. A brief, typed, essay (no more than 3 pages) discussing your plans to incorporate your academic work and degree at SF State with service in the HIV community or in the area of HIV prevention. (Please use additional sheets of paper and attach to the application form.)

We make every effort to protect the confidentiality of applicants and the recipient of the scholarship.

Note: All information collected as a part of the application process will become property of the Scholarship Committee, will be kept confidential and will only be used by the committee for the purpose of evaluation of applicants for the Westwood Scholarship.
