Registering with the DPRC
Register with the DPRC and document your disability to help you fully access student life at SF State using the following steps.
- Apply to the university
- Complete a DPRC registration form and request your accommodations
- Obtain disability documentation
- Submit DPRC registration form and disability documentation
- Meet with a DPRC specialist

Apply to the University
- All prospective students need to apply to the university through the standard admission procedure found at CSU Mentor before registering with the DPRC. Application dates and deadlines as well as the online application are also posted on CSU Mentor.
- The freshman and transfer admission criteria can be found at the same website.
- If you are denied admission to SF State and believe that you have been unable to complete a subject requirement because of a disability, an appeal can be made to the SF State Undergraduate Admissions Office. It is recommended that you include with your appeal disability documentation by a qualified professional illustrating how the disability has affected your academic performance. Complete the DPRC Registration Form and Request Your Accommodations
The online DPRC registration form allows you to tell us more about your disability and the reasonable accommodations you’d like to request. If you are not sure what certain accommodations are, don’t worry. You’ll get a chance to ask when you meet with a DPRC Specialist. After completing the online registration form, you will need to print it out, sign it, and submit it to the DPRC.

Obtain Your Disability Documentation
The DPRC welcomes and encourages all students with disabilities at SF State to register with us. In most cases, we will ask you for disability verification from a qualified professional. Documentation requests are meant to help us understand as much as we can about your disability in advance so that you can focus on being a student. Please note that disability documentation that may have been sufficient to establish eligibility for reasonable accommodations at other institutions, such as high schools or community colleges, may not automatically transfer and may need updating before establishing accommodation eligibility at SFSU. You may not need to submit disability documentation if your disability and accommodation requests can be verified by observation (e.g., wheelchair user, etc.)
Prospective DPRC Students with Disability Documentation Questions
Please note that prospective DPRC students who would like to meet with DPRC specialist before submitting disability documentation may do so by contacting the DPRC front desk at 415-338-2472 in the Student Services Building, room 110, to schedule an appointment. Students may want to do this if they need DPRC assistance in obtaining disability documentation, if they believe their disability and accommodation needs can be verified by observation, or for personal reasons unique to their individual situation. Provisional DPRC services may be available while documenting your disability on a case-by-case basis.
Documentation guidelines
Below are documentation guidelines for you and your qualified professional. Please use these guidelines when obtaining your disability documentation. Generally, the more specific your documentation, the faster we can process your accommodation request and set up a time to meet with you. In the event you are not able to obtain disability documentation that meets all elements stated within the guidelines, you should still submit as much as you have.

Submit Form and Documentation
Completed registration forms and disability documentation are reviewed in the order received. Submit to:

Your registration form and disability documentation will be assigned to a DPRC specialist for an initial review. At the same time you will receive an email message confirming your submission has been received that also indicates the name of the DPRC specialist your submission has been assigned to, and how to contact the specialist if you have any questions.
Meet With a DPRC Specialist
After an initial review of your disability documentation and registration form, you will be contacted to set up an initial appointment with a DPRC specialist. Our goal is for the intake appointment to occur within three weeks of the date we received both your disability documentation and your registration form. At this appointment the specialist will conduct an interactive process with you that includes:
- Clarifying your rights and responsibilities as a student with a disability at SF State
- Arranging the reasonable accommodations and services you have been found eligible to use
- Interviewing you regarding any accommodation requests that may not be fully supported by the disability documentation on file
- Establishing a plan for obtaining additional documentation if necessary
- Answering your questions about the reasonable accommodation process
Many accommodations can be set up within the intake appointment to be used immediately (e.g., shuttle service, parking). Some accommodations (e.g., exam accommodations, note-taking) may take a week or two implement in order to coordinate scheduling. Other accommodations may take longer to arrange. When requesting the accommodations listed below, please allow a minimum of four weeks preparation and coordination time.
