- If completing an 895, a signed copy of the abstract page must be attached to the Report of Completion.
- Must be signed by both committee members and the Department Chair indicating that your Culiminating Experience has been completed (895/892/890/written and/or oral comprehensive exam). If applying for graduation, the "date completed by" must be no later than the last day of instruction for the semester you applied.
