Procedures for Completing Undergraduate Application for Graduation
1. Pick up an application from the
One Stop Student Services Center in the Student Services Building
during the first week of your final semester (or summer session)
or access it online.
The completed application must be submitted by the deadline
for the term in which you expect to graduate. NOTE:
Finance, Banking and Hospitality Management majors must visit
the department office in BUS 314 before completing the application.
2. Type or print neatly in ink if using
the paper form, or follow online instructions. If you use
the online form, select the listing that indicates your concentration
or emphasis, i.e. Bus Admin (Accounting) 400307.
a. The first section contains personal information; information
about Segment III and JEPET, and asks you to list all courses
in progress or to be completed.
b. In the second section, LIST ALL THE COURSES CONSTITUTING
YOUR MAJOR in the order in which they appear in the Bulletin*
and Core Course Sheet. List all courses in progress or to
be completed, but leave the grade blank. Do not list prerequisite
courses, ENG 114 or 214, SPCH 150, ECON 100 or DS/MATH 110
or equivalents.
c. List courses taken at institutions other than SFSU with
the department abbreviations, numbers, titles, units, and
grades as they appear on your transcript from that institution.
You need to have equivalencies on a Course Substitution Form
(pink sheet) which you will attach to the application. Sign
the application and keep a copy for your records.
d. Don’t have a Course Substitution Form? Bring a copy
of your non-SFSU transcripts to BUS 137 for help.
3. Meet with a Faculty Advisor from
your major department for their approval and signature. Faculty
office hour information is posted outside BUS 137, outside
of department offices, and online.
4. Pay the application fee at the Cashier’s
Office before the deadline. ($40.00 for first time applicant;
$10.00 for re-applications; an additional $5.00 late fee charge).
A paid stamp will be imprinted on your form.
5. Turn in the completed and signed
application to BUS 137 for the Department Chair’s signature.
NOTE: Finance, Banking and Hospitality
Management majors turn in your application to BUS 314.
Your major department will then review your application and
forward it to the Registrar’s Office for processing.
You will be contacted by the major department if there are
any problems with your application. The Registrar’s
Office will issue a preliminary report once they have reviewed
your application. If you have not received a preliminary report,
contact the Degree Evaluation Office directly at (415) 338-1634.
6. Application denied? If your graduation
application is denied because you have not finished all course
work or graduation requirements, you must re-apply for graduation
and follow the procedures listed above.
*Bulletin Requirements and Rights
Students may choose between three Bulletins:
-- The one in use when the student began attendance in regular
sessions in any combination of CSU and California Community
College campuses (requires continuous attendance, which is
defined as enrollment in at least one semester each calendar
year);
-- The one in use at time of transfer to SFSU; or,
-- The one in use at the time of graduation.
Effective Fall 1986, students who declare/change majors are
required to follow the Bulletin requirements for that major
in effect at the time of the declaration/change or in effect
when the student graduates.
