CGLab registration/contact form (contact.pdf) - Contact and billing information for new users and sequencing submitters. Please download, print, fill out, and submit to CGLab.

CGLab monthly charge tally form (charges.pdf) - please download, print, fill out, and submit as a record of your monthly use of CGLab consumables, special orders, etc. If you did not work in CGLab or use CGLab materials in a given month, please inform us of that by email (and no form is needed).

CGLab sequencing request form (sequence.pdf) - This form includes all information required for CGLab contract sequencing, including: template identifiers, fragment size and concentration, cleaning method, primers used for sequencing, annealing temperature of PCR, etc. Please download, print, fill out, and submit to CGLab with each batch of materials for sequencing.

Certificate of Safety Orientation (safety.pdf) - must be submitted once yearly to maintain your safety training registration. Please download, print, fill out and submit to CGLab.
