Delivery/Pick-up Service Information

We will only deliver multimedia carts and 25" TV/VCR combos. Please pick-up all other equipment in person or arrange for a semester loan (Click for details).
In person equipment pick-up options:
Faculty may come in person to the AV counter and sign out the equipment.
Faculty may come in person to the AV counter and PRESIGN the forms for a student to pick up at a later date.

Fall 2008 Important Dates:
First Day of Instruction:	Tuesday, August 26, 2008
First Day of Delivery/Pickup Service:	Tuesday, September 2, 2008
Labor Day (Çampus Closed)	Monday, September 1, 2008
Daylight Saving Time ends:	Sunday, November 2, 2008 (2am)
Fall Recess:	November 24-28, 2008
Last Day of Instruction:	Monday, December 15, 2008
Final Exams:	December 16-22, 2008

Requests must be received by the AV Service Desk no less that three (3) full working days before the service date.

Service will be provided only if student assistants are available.
You need to fill out a separate DELIVERY/PICK-UP FORM for EACH DAY you need equipment.
Forms are available at the AV Desk or as a PDF file on-line (please read the instruction page).

PLEASE PRINT AS CLEARLY AS POSSIBLE AND PROVIDE A TELEPHONE NUMBER WHERE WE CAN CONTACT YOU WITH ANY PROBLEMS OR QUESTIONS.

DELIVERY INFORMATION

You must be in the classroom to sign for the equipment. You are accepting full financial responsibility for all delivered equipment/media.
IF YOU CANNOT BE THERE, YOU WILL NEED TO PRESIGN EACH DELIVERY/ PICK-UP FORM IN ADVANCE AND ASSIGN SOMEONE TO COSIGN FOR YOU. CALL AV FOR MORE INFORMATION AT 81494.

We will wait 15 minutes past the scheduled delivery time. If you do not arrive, our student will return the equipment to AT. You will then need to come to our office to sign it out in person.
Our student assistant will set up the equipment for you (excluding camcorders).
Please note: The students who set up this equipment are not familiar with the software programs. It is the faculty or presenter's responsibility to be familiar with the programs they use.

PICKUP INFORMATION

Do not leave any equipment or media unattended. Do not lock it in the classroom and leave.

If you are finished before your scheduled pickup time, call 81494 to see if we can pick it up early. If no student assistant is available, you or one of your students must return it to our office.

YOU WILL BE HELD RESPONSIBLE FOR ANY MEDIA OR EQUIPMENT LEFT UNATTENDED BEFORE PICKUP TIME.
