How to Show Proof of Health Insurance Coverage - Instructions for F1 Students
Choice #1: CSU Sponsored Health Insurance
The CSU (California State University) sponsored health insurance plan meets ALI’s requirements. The insurance plan is offered by Anthem Blue Cross Life and Health Insurance Company, through Wells Fargo of California Insurance Services.

Choice #2: Other Health Insurance
If you choose not to buy the CSU sponsored insurance and you want to use your own insurance, you must have your insurance company fill out the Health Insurance Affidavit Form. Your insurance company can fax or mail it to the ALI.
By signing the affidavit form, your insurance company, is responsible for confirming that your insurance plan meets ALI’s insurance requirements.
Required Length of Health Insurance Policy
When you study at the ALI, you must have health insurance that covers you for entire semester.
Medical Evacuation and Repatriation
Some insurance plans do not cover medical evacuation and repatriation but ALI requires this coverage. If you need this coverage, you can buy Well Fargo of California Insurance Services’ “On Call International” at a reasonable cost.
