A new secure encrypted online version of the Address Change form is now available.

currently on file at SFSU, please feel free to try the
Address Change/Verification Form.
The student forms listed below can be printed. After all necessary signatures and
associated fees have been paid, you can return the form to the One Stop Student Services Center (SS 101) or mail the completed form to: 1600 Holloway - SF, CA 94132. Some forms can also be
faxed to One Stop at (415) 338-6703.
Forms Available for Current and Former Students
Academic Standing Petition
Mandatory Academic Probation Advising is required for all Undergraduate students who are on academic probation status or subject to disqualification. Mandatory advising is required for continued enrollment.
Signatures are required.
Add Form
Add Form is used for add classes and change grading option for the current semester.
Signatures are required, as well as tuition payment.
Address Change
Local Address Change can be done on MySFSU ASE Request
Advanced Standing Evaluation is a transfer credit evaluation. If you have an ASE, use it together with the Degree Audit Report (DARS) to get a complete picture of progress made toward your degree.
Credit By Exam
Matriculated students may obtain Credit By Examination for specific courses as determined by each department and approval by the course instructor and department chair. Interested students should inquire with each department policy.
Signatures are required as well as tuition payment.
Diploma Reissue
Request for Reissue of Diploma
The student signature is required, as well as payment in the form of a check or money order.
Baccalaureate Degree Completion Plan
The Baccalaureate Degree Completion Plan is only needed if a student is contacted by the Registrar's Office. See form for complete instructions.
Graduation Application
The Baccalaureate Application for Graduation forms are available online. Please read
all of the instructions prior to completing the forms. Signatures and payments are required.
Graduation Authorization To Change Major / Minor
This form is an Authorization for a Change to the Approved Major or Minor Program on the current Baccalaureate Degree Application.
Department approval is required.
Graduation Denial
Request for Release of Registration Due to Graduation Denial is to be used by student who did not receive priority registration as a result of filing for graduation.

ID Number Change
Student Number Change Request requires name and SFSU ID#.

Leave of Absence
Planned Educational Leave for continuing students who have been officially admitted.
Student signature is required, as well as departmental approval. Return form to the Registrar's Office, SSB-101.
Major/Minor Change
The Request for Change of Undergraduate Major/Minor or Addition of Secondary Major or Minor
requires department chair or head of program signature.
MMR / HEP B
Measles, Mumps, Rubella (MMR) Mandatory immunizations are required before students may register for classes. Students 18 years or younger are required to show proof of Hepatitis B Vaccine. MMR Waiver
Measles/Rubella Immunization Waiver allows students to request an exemption due to medical reasons, religious or personal beliefs. Name Change
Name Change Request is to be completed by student with proof and return to the Registrar's Office, SSB-101. Transcript Request
Transcript Request Form is to be completed by student. Student may pick up 2 sets of officical transcript at SSB-101 (Pick up may not be available for student admitted prior to 1985).

Petitions
Withdrawal Card
Notice of Withdrawal Card (Pre-Census: before 4th week) is a total withdrawal from the university for the current semester only.
Student signature is required.
Petition for Withdrawal
Petition for Withdrawal from Course(s) or University (Post-Census: after 4th week) procedure is recorded on the student's academic record as "W" grade.
Student signature is required, as well as department approval.
Petition for Retroactive Withdrawal
Retro-Withdrawal (Previous Semester) is used for withdraw from a prior semester with a valid reason, supported by documentation.
Student signature is required, as well as department approval.
Petition for Grade Change/Make-up Incomplete
Grade Change / Make-up of An Incomplete Form is used to change an incorrection grade to the correct grade or report the make up of an incomplete grade.
Instructor and departmental approval is required. Return forms to the Registrar's Office by the department.
Petition for Waiver of College Regulation
Petition for Waiver of College Regulations can be used for many different situations.
Student signature is required, as well as other appropriate signatures
(depending on situation).
Petition to Exceed Semester Maximum Unit Limit
Petition to Exceed the Maximun Unit Policy for Undergraduate Students request for 20 or more units, must be approved by the Major Advisor and Dean of the College. Undeclared and Special Majors must obtain the approval of the Dean of Undergraduate Studies (ADM 447).
Signatures are required.
Please refer to the online Bulletin under "Maximum Units in One Semester" for further rules and regulations concerning this form.
Petition for Course by Individual Study
Petition for 699 or 899 Course by Individual Study must be accompanied with an add form providing the 5 Schedule number by the department.
Department approvals are required. Return both forms to the Registrar's Office.
Petition for Incomplete
Petition for Incomplete allowed time to make up an incomplete grade is one academic year. Instructor may set a deadline earlier than the two-semester limit.
Department approval is required. Return form to department.

Concurrent/Visitor Enrollment
Application for Intrasystem Concurrent / Visitor Enrollment is to be used by California State University students who wish to enroll concurrently or as visiting students at another CSU campus.
Department approval and tuition payment is required.
Consortium Enrollment
Application for The San Francisco Consortium is to be used by SFSU student who wish to enroll concurrently (one course per semester only) with the San Francisco Consortium member of other institutions, listed on the form.
Department approval and tuition payment is required.
Cross Enrollment
Application for Cross Enrollment is to be used by California State University undergraduate students who wish to enroll concurrently in a California Community Colleges or the University of California (one course per semester only).
Department approval and tuition payment is required.
Cross Registration
Application for Cross Registration with UC Berkeley is to be used by SFSU student who wish to cross enroll concurrently (one course per semester only) at UC Berkeley.
Department approval and tuition payment is required.

Nonresident Tuition Exemption
Effective January 1, 2002, Assembly Bill (540) creates an exemption from payment of nonresident tuition for certain nonresident students who have attended a California high school for a minimum of three years and have graduated from high school(or attained the equivalent High School Equivalency Certificate issued by the California Department of Education).

CSU Request for Transfer of Admission Application to Alternate Choice of Campus
for use by CSU applicants who have already been accepted for consideration and wish to be considered
by another CSU campus. This form requires your name and SFSU ID Number.
