This form is for graduating students to request access assistance for themselves or their guests at the main commencement ceremony. Please complete the form below and submit by May 7, 2008. Information packets will be mailed out in May.

Please note: This form is NOT for tickets. Each guest must also have a ticket (which are given out when your rent your regalia).
