Assistance Request Form

This form is for graduating students to request access assistance for themselves or their guests at the main commencement ceremony. Please complete the form below and submit by May 7, 2010. Information packets will be mailed out in May.

If you have any difficulties completing the form, contact the Disability Programs and Resource Center or 415-338-2472 (voice/TTY).

Please note: This form is NOT for tickets. Each guest must also have a ticket (which are given out when your rent your regalia).
