It is the policy of San Francisco State University that the appropriate Cabinet Level Officer and the President approve all acquisitions of cellular telephones. Final approval will rest with the University President. SFSU Cellular Telephones Policy and Procedures
Procedures Telecommunications Services has the central coordinating responsibility for all cellular telephone transactions. Included among its responsibilities, is the primary one to insure that all requisite procedures are followed.
- For Acquisition and Installation
A. Obtain advice regarding the model/style of cellular telephone or adjunct equipment through IT Business Services at x82405 or email busserv@sfsu.edu. B. Complete the university Cellular Telephone Authorization Form and submit to the proper authorities for approval. C. After the form has been returned with approval, send it to IT Business Services at ADM 101. D. Upon receipt of the Cellular Telephone Authorization Request, IT Business Services will contact the cellular telephone vendor and arrange for a new cellular telephone number, an installation date, training for the user (if required), and the programming of the cellular telephone instrument. E. Upon completion of the telephone number installation, IT Business Services will establish an internal SFSU billing account for the cellular telephone user. All future service and usage will be paid by IT Business Services. Costs will be recharged through the University's telephone billing system.
- For Disconnection
Email IT Business Services at busserv@sfsu.edu to disconnect and specify the required disconnection date. IT Business Services will close out the cellular telephone account. The department can retain the cellular instrument.- For Telephone Number Change
Email IT Business Services at busserv@sfsu.edu to change numbers and specify the date requested. IT Business Services will contact the cellular telephone vendor to change the telephone number and notify the user of the new telephone number assigned and the programming appointment date. IT Business Services will follow up to insure completion of the number change and proper billing.- For Repair
Email IT Business Services at busserv@sfsu.edu. IT Business Services will determine the source of trouble and notify the user of the resolution of the problem or the estimated time and cost of repair. IT Business Services will follow up to insure that the problem has been resolved.- For Reporting A Stolen Instrument
Notify the Department of Public Safety (or the applicable police agency) and file a report. Obtain and record the police report number. Notify IT Business Services at busserv@sfsu.edu. IT Business Services will notify the cellular telephone company to arrange for either suspension or discontinuance of service.
