Online Delivery/Pick-up Request .PDF FORMAT

Due to a decrease in available student assistants, we will not deliver the following single items: overhead projectors, slide projectors, CD/cassette players. Please arrange for a semester loan (click for details) or pick-up in person.
In person equipment pickup options:
1. Faculty may come in person to the AV counter and sign out the equipment.
2. Faculty may come in person to
the AV counter and PRESIGN the forms for a student to pick up.

  1. This is not an e-mail request. You fill out the form on your computer, print it on your printer and bring it to the AV Center.
  2. Please do not print out a blank form and hand write the information. We will not be able to read it.
  3. This form has been designed to print on one side of an 8 1/2" x 11" white page. Do not use colored or legal size paper.
    While we understand the need to recycle whenever possible, these are business forms and should printed on unused paper.
  4. SYSTEM REQUIREMENTS: You will need Adobe Acrobat Reader 5.0 or above. Download it here.
  5. It is the responsibility of the faculty to check each form after it prints for accuracy of time, date, day of the week and equipment requested.
  6. All information must be clear and fit on one side of an standard sheet of white paper. The AV Center reserves the right to return or refuse improperly filled out forms.
  7. No copies will be made by the AV Center. Faculty must either print 2 copies or Xerox copies for their records.
  8. This form has been checked using Mac OS X and Windows XP. There have been reports that the fields are not accessible when the form is viewed from within the browser. Control-click on the "I Accept" link below and select "Save link target to disk." The form will download to your computer. Double click on the downloaded file and it will open with Acrobat Reader. The fields will then be accessible. This may solve accessiblity issues with other systems.

  1. All fields are in the top third of the form are required.
  2. A telephone number where you can be reached is very important.
  3. Type in the date as mm/dd/yy.
  4. Click in the boxes for the equipment you need.
  5. Fill in the AV Number and Title of any media required.
  6. IF YOU HAVE A T.A. WHO WILL BE ACCEPTING RESPONSIBILITY FOR THE DELIVERY, TYPE THEIR NAME IN THE COSIGNER FIELD.
  7. Print the form. PRINT AT 100%, DO NOT SCALE DOWN. Make sure you print (or Xerox) an extra copy for your records.
  8. For faculty who use the same equipment for every class, click the "Clear Date" button and change date and weekday for EACH DELIVERY YOU NEED.
    YOU MUST PRESENT ONE FORM FOR EACH DELIVERY/PICKUP YOU REQUIRE.

    Click "Clear All" to clear the entire form.
  9. SIGNATURE IS REQUIRED AT TIME OF DELIVERY ONLY. DO NOT SIGN THE FORM IN ADVANCE UNLESS:

  10. Bring the completed forms to the AV Service Desk in BH 24 for processing.

Please read the following information:

"Delivery Time" is the time equipment is required to be in the classroom and ready for use. A faculty member must be in the classroom within fifteen (15) minutes of the specified delivery time to sign for the delivery before the equipment can be left. After 15 minutes the equipment will be returned to the AV Center.

YOU WILL BE HELD RESPONSIBLE FOR ANY EQUIPMENT THAT IS LEFT UNATTENDED BEFORE THE PICKUP TIME. Do not lock it in your classroom and leave. If you are finished before the scheduled pickup time, please call 81494. If a student assistant is available, we will send them out. If not, you are responsible for returning the equipment to the check-in counter at the AV Center.

"Cosigner" is when the faculty will not be in class and wants another person to cosign for the equipment at the time of delivery. We will not leave any part of the requested delivery unless the form is presigned by the faculty and a cosigner's name is written on the form. (Only faculty can request this special service.)

In the classroom, our student assistants are there to assist the Faculty in a basic set up of the delivered equipment (excluding camcorders). In order to maximize our delivery/pickup services our student assistants will not stay to operate any AV equipment. It is the responsibility of the Faculty to be familiar with the equipment/software. Please call 81494 to schedule any training on the use of our equipment. We do not provide training for the use of software programs.

If you experience any technical problems with the equipment call the AV staff at ext. 81494. In some cases the problem can be resolved over the phone. Otherwise, an AV assistant will be sent to troubleshoot the problem.
